Student Affairs Survey Policy

All surveys sent out via SGSM will have to be approved by Student Affairs (unless 199 or less or workshop feedback – see SGSM Survey Policy)

There is a four-week lead time for review of the survey by Student Affairs

SGSM Survey Policy

Any survey that is 199 students or less (internal SGSM only) or for a workshop feedback (administered within 24 hours following the workshop) must be reviewed by the Executive Director for approval

There is a three-business day lead time for review of the survey

If you had to have your survey approved by Student Affairs, please send proof of approval to the Executive Director at svangink@uci.edu for further marketing via SGSM
 
 
 

SGSM Survey Policy

Surveys WITHOUT a Raffle Component

If your survey DOES NOT have a raffle component, you can use Google Forms, forms on the ASUCI website, Qualtrics, or the SGSM Survey Website.

SGSM Survey Website (survey.studentgov.uci.edu) will require pro-staff to set-up. You will need to submit a Service Now ticket two weeks in advance. If you are trying to restrict to a certain population, it’s suggested that you have pro-staff set-up the survey for you (by UCInetID, only undergraduates, etc).

You do not have to use pro-staff to set-up these types of surveys, only if you need assistance with any of the requirements above.

Surveys WITH a Raffle Component

Please ensure to complete the appropriate documents for the prize with the Business Office, i.e. gift cards

If your survey DOES have a raffle component, pro-staff will have to set-up the survey for you. You will need to submit a Service Now ticket two weeks in advance. Recommend placing Service Now ticket at same time of submitting for Student Affairs review.

By default, all surveys with raffles created by pro-staff will be done through the SGSM Survey Website (survey.studentgov.uci.edu) . If you specifically want it in Qualtrics so that you can use their features to model the data, you will need to create the survey in Qualtrics and share it with pro-staff. We will then copy it over to the main account that we use for Qualtrics. We will give instructions on how to do this.

Please either submit a document laying out the survey (please notate single line text, multiline text, multiple choice, etc) or create the survey in Google Forms and submit a link so that we can copy it over.

Include any restrictions for the survey. Undergraduates only, Graduates only, school populations, etc.
